If you have registered on-line, you need only print the registration/waiver form and fill-in the camper name along with your name (parent/guardian) and signature on the waiver line at the bottom of the form.

Most of you probably have the adobe pdf reader installed on your computer.  The pdf version will format better on your printer, but either form will do.  To print a form, click on one of the two links below.
